Connie Rosenberg, MPS,RN,CCM,CMC
PRESIDENT
She was elected President of the Greater New York Chapter of NAPGCM in 1992 & 1994. Connie is currently on the Board of Directors of the Guardianship Association of New Jersey . Connie was the 2003 recipient of the Adele Elkind Award for Professional Service in Geriatric Care Management at the Local and National Levels.
In 2011 Connie is returning to the Board of Directors for NAPGCM.
RICKI WEISER, MSW, LSW, CMC
ASSISTANT DIRECTOR OF CARE MANAGEMENT
Ricki has an MSW in social work with a concentration in Aging, Health & Mental Health. She has a post-graduate certificate in Gerontology and is a licensed social worker. Prior to joining Services and Resources for Seniors in 2002, her work experience included comprehensive social work services at several long term care facilities including Alzhemier/Dementia special care units. In addition, she has extensive experience as a Hospice social worker. Ricki is certified as a geriatric care manager by the National Academy of Certified Care Managers.
LISA MONDAY, MSW, LCSW, CMC
ASSOCIATE CARE MANAGER
Lisa has an MSW in social work and is a licensed clinical social worker. Prior to joining Services and Resources for Seniors in 2004 Lisa had been practicing for over 15 years in psychiatric and medical social work. She has extensive experience in emergency services, outpatient counseling , social work in a home care setting, and recreation therapy services in a nursing home setting. Additionally, Lisa was program director of a large older adult social group. Lisa is certified as a geriatric care manager by the National Academy of Certified Care Managers.
JACQUELINE ORNSTEIN RN, BSN
ASSOCIATE CARE MANAGER
Jacqueline is a Bachelor's prepared Registered Nurse and in addition has done graduate study in Community Health Nursing. She has a Certificate in Holistic Nursing. Prior to joining Services and Resources for Seniors in 2007, Jacqueline had been both a nurse case manager and a program director in an adult day care center in New York for nine years. In addition, she worked as a geriatric care manager in New York for six years.
ELYSE WEBER-SACKS MSW, LSW, CMC
ASSOCIATE CARE MANAGER
Elyse has an MSW in social work and is a licensed social worker. Prior to joining Services and Resources for Seniors in 2008, Elyse’s experience included work with a home care agency, a hospital where she was the social work supervisor of a rehabilitation unit and a hospice doing Provider Relations. In addition, she has been the Director of Community Relations for a skilled nursing facility. Her role included managing the admissions process for the elderly coming from hospitals and the home setting. Elyse is certified as a geriatric care manager by the National Academy of Certified Care Managers.
FRANCINE ROSSOFF, RN, BSN
Fran is a Bachelor’s Prepared Registered Nurse with over 15 years of visiting nurse experience. In addition, she has extensive experience providing high-tech nursing care in the home. Fran has done nursing visits for Services and Resources for Seniors, Inc. since 2005.
