Connie Rosenberg - Geriatric Care Manager

Connie Rosenberg, President of Services and Resources for Seniors, Inc., has provided private care management services for seniors and disabled adults throughout Northern New Jersey since 1987. She was elected to the Board of Directors of the National Association of Professional Geriatric Care Managers (NAPGCM) in 1991 and was the 2001 President.

She was elected President of the Greater New York Chapter in 1992 & 1994. In addition, in 1989 she was appointed Chairperson of the Standards and Ethics Committee of NAPGCM and continues to be an active member of the Committee.

RICKI WEISER, MSW, LSW, CMC
ASSISTANT DIRECTOR OF CARE MANAGEMENT

Ricki has an MSW in social work, a post-graduate certificate in gerontology and is certified as a geriatric care manager by the National Academy of Certified Care Managers. Her experience includes comprehensive social work services at several nursing homes including Alzhemier/Dementia special care units. In addition, she has extensive experience as a Hospice social worker.

LISA MONDAY, MSW, LCSW
ASSOCIATE CARE MANAGER

Lisa has an MSW in social work and is a licensed clinical social worker. She has been practicing for over 15 years in psychiatric and medical social work. She has extensive experience in emergency services, outpatient counseling , social work in a home care setting, and recreation therapy services in a nursing home setting. Additionally, Lisa was program director of a large older adult social group.

FRAN ROSSOFF, RN, BSN

Fran is a Bachelor’s Prepared Registered Nurse with over 15 years of visiting nurse experience. In addition, she has extensive experience providing high-tech nursing care in the home. Fran does as-needed nursing visits for Services and Resources for Seniors, Inc.

JACQUI ORNSTEIN RN, BSN
ASSOCIATE CARE MANAGER

Jacqui is a Bachelor's prepared Registered Nurse and in addition has done graduate study in Community Health Nursing. She has a Certificate in Holistic Nursing. Jacqui has been both a nurse case manager and a program director in an adult day care center in New York for nine years. In addition, she worked as a geriatric care manager in New York for six years prior to joining Services and Resources for Seniors, Inc.

ELYSE WEBER-SACKS MSW, LSW
ASSOCIATE CARE MANAGER

Elyse has an MSW in social work. Her experience includes work with hospice, a home care agency and in a hospital where she was the supervisor of a rehabilitation unit. In addition, she has been the Director of Community Relations for a skilled nursing facility. Her role included managing the admissions process for the elderly coming from hospitals and the home setting.

Services and Resources for Seniors (SRS) is designed to help older and disabled adults and their families to develop specific plans of care. SRS focuses on the client and family and seeks to blend the loving care of the family network with the available resources in the community.

Together we will consider the concerns of the family and the limitations of the older/disabled person to develop and help to implement practical solutions for problems of daily living.
